What is Personal Action Towards Health (PATH)?
PATH is a workshop that empowers people to take an active role in managing a chronic or long-term health condition.
Who Should Participate?
Adults with any chronic or long-term health condition including arthritis, heart disease, diabetes, emphysema, asthma, bronchitis, and depression. Family members, friends, and caregivers are also encouraged to attend.
